Tab 14: Conflict of Interest Policy and Disclosures

The community foundation submits a board-approved conflict of interest policy that covers all aspects of its operations and applies to board, staff, and standing board committee members.

 

Tab 15: Whistleblower Policy

The community foundation submits evidence that there is a system to receive and respond to internal and external good faith complaints about violations of the law or of the ethics or organizational policy or other misconduct by trustees, directors, or staff members.

 

Tab 16: Written Record Retention Policy (Electronic and Paper Documents)

The community foundation submits a written policy that includes retention guidelines for covered documents, guidelines that indicate when it is permissible or mandatory to destroy applicable documents, and the procedures for ceasing destruction of documents.

 

Tab 17: Human Resources Documentation

The community foundation submits the Chief Executive Officer (CEO) job description; evidence of regular CEO reviews, including that nature and extent of the review; and the process for determining the CEO’s compensation. Additionally, the foundation submits evidence of a board resolution that the foundation’s personnel policies shall comply with federal, state, and local requirements.

 

Tab 18: Confidentiality Policy

The community foundation submits documentation that describes the policy and/or procedures used to ensure that information is kept private and confidential. The policy applies to both staff and board members.

 

Tab 19: Sample Disclosure Documents

During the gift planning process, the community foundation discloses the roles and relationships of all parties involved. Examples include disclosure of the community foundation’s relationship to a financial institution, attorney or financial planner in situations where those facilitating the donations are receiving fees paid from the donors gift.

 

Tab 20: Oversight of Geographic Affiliates

The foundation submits evidence of oversight of geographic affiliates. For purposes of this Tab, geographic affiliate is defined as a component fund (or collection of component funds), established within or by the community foundation, serving a defined geographic region and under a common advisory group.
